The psychiatrists 　that accompanied　 my mission said that his state of health is critical, and if he did not get urgent relief, that we would have to expect a rapid deterioration　of that state of health, and possibly with irreparable harm.
If he did not からはじまる文は　仮定法過去の文でしょうか？

はい、仮定法過去だと思います。
仮定法過去は「if + S + 動詞の過去形, S + would(could, might等助動詞の過去形) + 動詞の原形」です。
If he did not get～の文では、”did”という過去形を使っており、その後のwe would have to ～の文でも”would”という助動詞の過去形の後に”have to”と原形が使われています。

ちなみに、この”have to”は「～しなければならないと」という必要を表します。
